The excellent care provided to residents of The Lutheran Home is also available to you in your home!  Home Health services provided by The Lutheran Home are designed for individuals living in their own home who need medically necessary care.  Studies have shown that patients often improve greatly after in illness or injury if they can receive medical care in their own home.  Our staff works closely with each patient’s physician to develop an individualized plan of care that best meets the needs of the patient.

We will gladly provide you – at no charge - with a telephone consultation with an experienced registered nurse (RN) who can discuss home health options and answer any questions you may have regarding home health.  Call 573-334-1515 for a consultation.

The Lutheran Home’s Home Health team includes:

·         Skilled Nurses – Registered nurses who provide a comprehensive physical assessment and coordinate with the patient’s physician to develop a plan of care to meet the patient’s needs.  A nurse is on call 24 hours a day, seven days a week.

·         Home Health Aides – Trained aides who provide personal care and assist with activities of daily living, supervised by a registered nurse.

·         Physical, Occupational, and Speech Therapists – Licensed therapists employed by The Lutheran Home who may provide treatments and modalities as ordered by the physician in order to promote recovery.

·         Medical Social Services – Counselors who may provide patient/family education and instruction related to personal and financial concerns.

You qualify for Home Health services provided by the Lutheran Home if you:

·         Are unable to leave your home without assistance

·         Have a need for skilled health care resulting from illness or injury that requires the care of a registered nurse or rehabilitation therapist

·         Have the care ordered by your physician

Home Health services provided by the Lutheran Home may be covered by:

·         Medicare

·         MO HealthNet (Missouri Medicaid)

·         Private health insurance

·         Workers’ Compensation

·         Automobile insurance
